Who is responsible for your information

The Fifth Sense Clinic is a trading name of FSCFSC Ltd, the data controller for the personal information held about you. The clinic is registered with the Information Commissioner’s Office (ICO).

Information collected

  • Your name, address, email address, phone number and date of birth
  • Your medical history, medications, allergies and consent forms
  • Clinical notes and treatment records
  • Clinical photographs taken with your consent
  • Payment records. Card details are processed by Stripe and are not stored by the clinic
  • Messages you send by email, WhatsApp or through this website

How information is used

  • To assess you and provide safe treatment. The lawful basis is the contract with you and for health information the provision of health care under Article 9(2)(h) UK GDPR
  • To take payment for treatment. The lawful basis is the contract with you
  • To meet legal and professional duties including record keeping. The lawful basis is legal obligation
  • To reply to enquiries. The lawful basis is legitimate interests
  • To send offers and clinic news by email or WhatsApp. This only happens if you have opted in. You can opt out at any time by replying STOP or by email

Photographs are never used for marketing without your separate written consent.

Who information is shared with

  • The clinic’s secure electronic medical records and booking system
  • Stripe, the payment provider
  • Email and WhatsApp services used to communicate with you
  • Your GP or another healthcare professional where needed for your care. Your agreement will usually be asked for first unless the law requires information to be shared

Personal information is never sold.

How long information is kept

Medical records are kept for 8 years after your last treatment. Enquiries that do not lead to treatment are deleted after 12 months. Marketing preferences are kept until you opt out.

Your rights

You have the right to access the information held about you and to ask for it to be corrected, deleted, limited in use or transferred to you. You can also object to how it is used and withdraw consent at any time. Some medical records must be kept by law even if deletion is requested.
